A hospital's relationship with its community is an integral part of its ability to serve residents and improve their access to healthcare. Since 1995, MultiPlan's Rural Health Outreach Grant Program has awarded over $520,000 to help rural hospitals reach out with programs and services that address the healthcare needs of their communities.
Faith Community Health System in Jacksboro, Texas will use their grant for their Wellness by Faith program, which provides health information presentations to members of the community. The presentations are led by health care professionals and focus on topics relevant to three targeted populations: women, Hispanic Women and senior citizens.
Stonewall Memorial Hospital in Aspermont, Texas will use the grant in conjunction with their Transition of Care program which provides much needed assistance to patients after they have been discharged from the hospital. As part of the program, the Transition of Care nurse will follow up with patients to ensure they understand their healthcare instructions, have necessary prescriptions and, if necessary, arrange for transportation for follow-up appointments.
Through MultiPlan’s Holiday Card Program we invite pediatric patients at hospitals participating in our networks to submit holiday-inspired drawings which are posted here on our website for a public vote. The top five drawings will be featured on MultiPlan's holiday card that we send to MultiPlan contacts across the country. In addition, the hospitals participating in this program will receive a supply of boxed holiday cards, courtesy of MultiPlan, for use in fundraising activities, at holiday events, or as gift shop items.
MultiPlan's Toy Car Program supplies battery-powered ride-on toy cars to pediatric units at participating hospitals. The cars give young patients a fun distraction to make their hospital visit a little less scary. For more information about our Toy Car program, contact us at email@example.com.
