Identify Your Network Logo

To correctly identify your healthcare network, select the logo displayed on your benefits ID card. If you see more than one matching logo on the front or back of the card, select the logo with PHCS in the name. You may select only one logo; however, if you also see the words "Healthy Directions" or "Extended PPO," please also select that option below.
