Thank you for your interest in joining MultiPlan’s provider networks (PHCS Medicaid Network or in Texas, the Texas True Choice Network) serving Medicaid and related government funded programs.

This application request applies to participation in the PHCS Medicaid Network or Texas True Choice Network only. You may already participate in one or more of our PPO networks (PHCS, MultiPlan, HealthEOS) that serve the privately-funded health care market, which are covered by a separate agreement. To request participation in these networks, visit our website at www.multiplan.com and click the “Join our Networks” button.

To complete the request form, first save it to your computer, complete the required fields, save the form and then send it to MultiPlan at the following email address: GBSNominations@multiplan.com.
