You currently have access to one or more of our national PPO network solutions. If you are not yet familiar with the network(s) available through your plan, please take a moment to read these short descriptions, then choose 'Search for a Doctor or Facility' to find participating providers. You will begin by telling us about the logo(s) on your member ID card. Click here for detailed instructions.

PHCS Network. A PHCS logo on the front of your member ID card indicates you have access to the PHCS Network. This is your primary PPO network. When you select healthcare providers from within this network, you pay the lowest out-of-pocket costs because your plan's co-pays and/or in-network coinsurance levels apply.

PHCS Healthy Directions. A PHCS logo on the back of your member ID card indicates you have access to the PHCS Healthy Directions network, which serves as your extended primary PPO network. You use PHCS Healthy Directions when you seek care outside your primary PPO service area (i.e., when traveling, studying or residing outside the area served by your plan's primary PPO). As with PHCS Network, your plan's copays and/or in-network coinsurance levels apply so your out-of-pocket costs are lowest when seeking care in the extended PPO. Note that sometimes, the words "Healthy Directions" or "Extended PPO" are displayed on the card instead of the logo, and in some cases these words may be on the front of the card.

MultiPlan Network. A MultiPlan logo, usually on the back of your member ID card, indicates you have access to the MultiPlan Network. This is a complementary network; that is, the MultiPlan Network is used as a complement to your health plan's primary PPO network. While your out-of-pocket costs will most likely be based on your plan's out-of-network coinsurance levels when selecting a complementary network provider instead of a primary PPO network provider, you get the benefit of the MultiPlan Network discount so your out-of-network benefit dollar goes further.

HealthEOS Network. A HealthEOS logo (with or without a plus sign) on the front of your member ID card indicates you have access to the HealthEOS Network. This is your primary PPO network. When you select healthcare providers from within this network, you pay the lowest out-of-pocket costs because your plan's co-pays and/or in-network coinsurance levels apply.

HealthEOS Select. A HealthEOS Select logo (with or without a plus sign) on the front of your member ID card indicates you have access to the HealthEOS Select Network. This is your primary PPO network. When you select healthcare providers from within this network, you pay the lowest out-of-pocket costs because your plan's co-pays and/or in-network coinsurance levels apply.
